What Is a Medical Declaration Form?
The Vietnam Medical Declaration Form is a new entry requirement for international travelers coming into the country. It can be completed online prior to the journey or a paper version of the form can be obtained at the point of entry to Vietnam.
This health declaration is necessary to visit both the mainland and Vietnam’s islands and coasts.
The online Medical Declaration Form is available in multiple languages, including Vietnamese, English, Chinese, Japanese, Korean, Cambodian, Spanish, French, and Italian.
It is recommended that international travelers fill in the health control form for Vietnam online within 24 hours before their flight is due to depart, if possible. Some airlines may not allow passengers to check in or board without a screen capture or QR code from an e-health declaration form in their name.
Do I Need to Fill in the Medical Declaration Form for Vietnam?
It is compulsory for all international travelers entering Vietnam to make a medical declaration, including passengers and airline crew. This is in addition to any required visas, such as the Vietnam eVisa.
Vietnamese nationals, foreign residents of the country, and even diplomats are all obliged to fill in the Medical Declaration Form.

Yes, quarantine is another requirement for traveling to Vietnam during the pandemic. All international arrivals must:
- Have a completed Medical Declaration Form
- Undergo medical screening upon arrival
- Quarantine for 14 days after arrival

Do I need to submit a PCR test before traveling to Vietnam?
Certain travelers must take an RT-PCR test before coming to Vietnam. Those who fall into the following categories are obliged to present a certificate showing a negative RT-PCR test result:
- Diplomatic or official passport holders traveling on duty
- Individuals traveling on business as experts
Those staying less than 14 days in Vietnam must have received their test result 3-5 days before arrival in the country.
Those staying longer than 14 days must have had their test result issued 3-7 days beforehand.
Vietnamese nationals and residents returning to the country are exempt from needing a test result, but must still have a Medical Declaration Form and comply with quarantine regulations.
Steps to Fill in the Medical Declaration to Travel to Vietnam
Completing the Medical Declaration for Vietnam is simple. Travelers must follow these easy steps:
- Go to the online Vietnam Health Declaration platform
- Select the language of preference
- Select the point of entry (airport, seaport, or land border crossing) to Vietnam
- Fill in the required information in the fields provided
- Click “Submit Form”
After this, the traveler will receive a unique code, which must be presented upon arrival in Vietnam. The individual should keep this code with them during their journey.
Alternatively, passengers may choose to wait until they arrive at the port of entry, go to the Medical Declaration Area, and fill out a paper form. However, some airlines may not permit individuals to board their flights without having already filled out the online health declaration.
What do I need to apply for the Vietnam Medical Declaration?
Travelers will need their travel details and passport number to complete the Medical Declaration Form for Vietnam.
If the individual has recently been tested for COVID-19, it is optional to upload the test results as part of the declaration.
Medical Declaration Form Questions
The Vietnam Medical Declaration Form asks the traveler to fill in some basic personal information, including the following:
- Date of birth
It also asks for travel information, including the following details:
- Passport number
- Port of entry to Vietnam
- Type of transportation (plane, ship, etc.)
- Transportation number and seat number
- Departure date
- Place of departure
- Destination in Vietnam
